Step #1: New Client Inquiry
To start the process of becoming a patient with Renew Institute, please complete the New Client Inquiry Form below. After submitting the form, you will receive a New Client Inquiry Call from a Renew Institute Client Relations Specialist. During the New Inquiry Call, a Client Relations Specialist will discuss our process and answer any questions you may have. Online consults & appointments available nationwide!
Step #2: Account Registration
Renew Institute uses an online portal to track all patient records, appointments, and communications. Immediately after your New Client Inquiry Call, you will receive an email containing the link to finish setting up your account in the Patient Portal. Once you’ve logged in to your patient portal, please complete all intake questionnaires located under “Questionnaires," and upload an image of the patient under “Labs & Documents."
Step #3: Scheduling Call
Once you’ve submitted all intake forms in the patient portal, you will receive a Scheduling Call from a Renew Institute Client Relations Specialist who will schedule your consult, address any missing information in your patient portal, and answer any questions you have.
